IMPORTANT INFORMATION FOR NEW PATIENTS
On the day of your appointment, please arrive 10 minutes before your scheduled appointment. It is required that you bring your Dental Insurance card with you if you have one.
A parent or guardian must accompany patients under 18 years of age and remain at the office during the length of the appointment.
Payment for dental services is due at the same time you receive the dental care. If you have dental insurance please refer to the Dental Insurance Information portion of the Patient Forms.

YOUR FIRST VISIT
Your initial appointment usually will consist of an examination and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:

Any x-rays taken by a previous dentist, if applicable.

A list of medications you are presently taking.

Written correspondence from your physician if you have had joint replacement, but no longer premedicate with an antibiotic prior to dental appointments

If you have dental insurance, please bring the necessary completed forms. This will save time and allow us to help you process any claims.
PRE-EXISTING MEDICAL CONDITIONS
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Please remember to take your antibiotic premedication 1 hour prior to your appointment if your physician requires this or we may have to reschedule your appointment to ensure your safety.
X-RAYS
Every new patient must have a FMX (full series of x-rays) of diagnostic quality that was taken within the last 3 years. If your dentist has taken x-rays, please request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they will be taken during your visit. New patients will still need 4 bite wing xrays, even with a current FMX, unless the bite wing x-rays were taken within the last 6 months.
INSURANCE
Brewster Dental Arts participates as an in-network provider with the following insurances companies:
1) Delta Dental Premier
2) Blue Cross Blue Shield- Dental Blue Indemnity Plan
Please Note:
Our participation in the BCBS Network
is ending on January 15, 2025.
As a courtesy to you, we will bill your insurance company and track claims (even if we are out of network with your plan) . Please keep us informed of any changes to your insurance plan.
You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
